Eligibility Requirements:
To be eligible for us to offer you a position, you must first provide us with the following documentation:
- A copy of your High School Diploma or GED if you received one. Original college transcripts will be
requested for all positions requiring a degree; however, a copy will be acceptable until a job offer is made; - A physician’s statement which, at a minimum, provides evidence of the absence of any indication of
active tuberculosis. Documentation of the Mantoux TB Skin Test results must be read within the past year and
be signed by a physician, family nurse practitioner, physician’s assistant, or registered nurse. (This initial
screening will be performed at no charge to you and must be provided by a physician approved by Lovebond
Community Outreach, LLC); - A copy of your current automobile insurance policy summary page or a certificate of insurance which
indicates (1) that you are a covered driver under the policy, (2) your insurance company’s name, (3) your
limits of liability, and (4) the effective dates of coverage.
